Paintball is one of the safest games you can play (check out this report), however, Outlaw Paintball requires all participants to have a release form on file before game play.

To speed up your check-in time when you come to play, download, print, and fill out the appropriate release forms for every single person in your group. Minors must have their forms signed by a parent or legal guardian. Everyone must have a release form, no exceptions.
